The Confidential Safety Reporting System is a program is designed to improve Rio Metro safety by collecting and analyzing reports which describe unsafe conditions or hazard and near miss within Rio Metro operations. Report safety issues or near miss incidents voluntarily and confidentially.
The information you provide will be used for statistical purposes only. Your responses will be kept confidential and will not be disclosed in identifiable form to anyone other than RMRTD employees or RMRTD agents such as telephone interviewers. Only statistical and non-identifying data will be made publicly available through reports. RMRTD will not release to FTA, or any other public or private entity any information that might reveal the identity of individuals or organizations mentioned in the unsafe conditions or hazard and near miss reports.
